Starting the cure

The episode opens with Sam, dressed as a doctor, watching a priest bless some blood, which he takes back to the bunker. Dean is tied to a chair in a Devil’s Trap in the dungeon. As Sam prepares to start the cure, Dean picks at Sam, trying to get under his skin. After Sam injects him with the first blood treatment, Dean has a violent reaction.

Meanwhile, Cas and Hannah drive out to the bunker. Hannah is worried about Cas, but Cas is worried about Sam — and that if the cure doesn’t work, the demon with Dean’s face will need to be dealt with.

Back at the bunker, Sam gives Dean another injection and Dean, again, reacts badly. After recovering, Dean again goads Sam with harsh words. He says he knows what Sam did while looking for Dean and questions which one of them was truly a monster.

It turns out, Sam found Lester at a bar and was the one to tell him about demon deals. Sam helped him summon a demon, but Lester made the deal for his wife’s life before Sam could stop him. Sam, though, still captures the demon and tortures her.

Sam injects Dean with another syringe of blood, and Dean reminds him of what he’s going to have to do if the cure doesn’t work.

Preparing for the worst

Meanwhile, Crowley holds court in Hell. One of his followers brings an update about Cas, and Crowley has the demon follow Cas. While Crowley reminisces about his BFF time with Dean, another demon offers to be Crowley’s wingman instead. Insulted, Crowley explodes him.

Cas and Hannah are forced to pull off on the side of the road because they’re lost. Though Cas isn’t doing well, he refuses to kill another angel or beg from Metatron. Cas and Hannah start to have a moment, but it’s interrupted when Sam calls. Sam is worried that the treatment is killing Dean. Cas, though, tells him to be prepared for what he may have to do.

Sam goes into the dungeon and revives an unconscious Dean. Dean continues to pick at Sam, telling him that his brother has been MIA for quite a while and that he’s sick of him. Sam, though, injects Dean and tells him, “This is me, yanking your lame ass out of the fire. You’re welcome.”

Back in Hell, Crowley has an Abaddon follower executed for treason. Another demon calls Crowley out on his recent behavior before dousing himself with holy oil and setting himself on fire in protest of Crowley’s rule.

Priorities

Back on the road, Cas and Hannah stop at a gas station. Cas awkwardly rejects Hannah’s interest in him, saying that feelings are temptations and they need to keep their priorities clear.

Hannah accepts this before going into the convenience store, only to find that the clerk’s eyes were burned out. Cas then enters to find Adina with a blade at Hannah’s neck. Adina throws Hannah into a shelf, and a struggle between Cas and Adina ensues with Adina easily taking the upper hand.

Back in the bunker, Sam goes into Dean’s room and finds some family photos under some half-eaten food and notes. Sam looks through them as sad music plays before heading back down to the dungeon — but Dean is gone.

At the gas station, Crowley finds a beaten down Cas. He heads into the convenience store, where Adina is torturing Hannah. Crowley cuts Adina’s throat, steals her grace, and stabs her. He then gives Adina’s grace to Cas, saying Cas is useless to him dead. After Cas heals, Crowley tells him to fix the problem of Demon Dean.

The game begins

In the bunker, Dean stalks Sam with a hammer while Sam grabs a set of keys and puts the bunker on lock down, bathing it in red-tinged darkness. Dean, though, says he’s not going to leave until he finds Sam. He says he was able to escape because the less demon he became, the less the cuffs worked.

When Dean finds the keys gone, he heads to the electrical room and takes the bunker off lock down, but Sam locks him in. He pleads with Dean to let him finish the treatments. Dean retorts that he doesn’t want to be cured and breaks down the door. Sam flees while Dean follows.

Sam peers around a corner, only to nearly have his head taken off by Dean. Sam puts the demon-killing knife to Dean’s neck, and Dean tells him to do it. But Sam drops the blade in time for Cas to grab Dean from behind. They take Dean back to the dungeon and continue the treatments.

Human again

Sam is bothered by Dean saying he didn’t want to be cured despite being almost human, and Cas replies that with all the pain humans feel, being a demon is probably easier.

Dean then comes to, his black eyes fading. When Sam throws holy water at him, there’s no reaction — Dean’s back, it seems.

Later, Sam finds Cas and asks him to keep an eye on Dean while he goes to get food. Cas reminds Sam that the Mark of Cain is still going to be an issue, but Sam just wants to fight one battle at a time.

Meanwhile, Dean looks through the pictures, but when Cas knocks, he hides them. Dean thanks Cas for stepping in and asks about Sam. Cas says Sam knows that whatever Dean said or did, it wasn’t all him and encourages Dean to take some time to heal.

Meanwhile, in Tulsa, a mysterious woman sits in a hotel room with two bellhops nailed to the ceiling.
