The CW has released the preview and a clip from the third episode of Supernatural‘s eighth season, entitled “Heartache.”

The clip gives us a potential double meaning for the title, with the figurative heartache that the brothers are respectively feeling and the literal heartache that comes from a case with hearts being ripped from chests. We also see that the brothers are still prickly with one another about how Sam spent the year between seasons 7 and 8.

According to the episode synopsis, the episode will focus on the brothers working a job “where the victims were all recipients of organs from the same donor.” Things will get complicated after the killer is captured because “the brothers find their killer in a trance mumbling an ancient prayer.”

The episode also marks the third Supernatural episode directed by star Jensen Ackles, after season 6’s “Weekend at Bobby’s” and season 7’s “The Girl Next Door.”
