Sarah Hyland, best known to television viewers as Haley Dunphy on ABC’s Modern Family, has filed a restraining order against ex-boyfriend Matthew Prokop.

Hyland says in a temporary restraining order obtained by TMZ that Prokop attacked her verbally and physically, at one point choking the starlet.

“His grip was so tight that I could not breathe or speak. I was scared and in fear for my life,” she said in the restraining order.

A breaking point occurred when Hyland asked Julie Bowen, her Modern Family co-star, to help end the relationship. When Hyland tried to give Prokop a plane ticket back to his homestate of Texas, he came over to Hyland’s home and threw a fit when he saw Bowen and others at the residence.

Hyland also says in the restraining order that Prokop threatened to burn one of her homes down and kill her dog. After the breakup, Hyland says that he “relentlessly bombarded me with vile, threatening and emotionally disturbing texts and voice mails including his own suicide threats.”

A Los Angeles judge has granted Hyland and her dog the temporary restraining order.

Prokop once appeared on Modern Family during the 2012 “Disneyland” episode. He’s also had roles in television (Hannah Montana) and film (High School Musical 3: Senior Year). Prokop and Hyland starred opposite one another in the 2011 television movie Geek Charming.

The two first met on the set of High School Musical 3 in 2009 and began living together in 2011.
